League Interest Form Fall 2026

Interested in joining a league, but need some assistance? 

Use this form if you are interested in:

• Looking for teammates and aren't comfortable with individual registration 
• Finding a player to split a league with
• Finding a team who can add a fifth player 

Fill out this form and our drawmasters will do our best to help you! 

**Note that this is not an official league registration form**  We cannot gurantee league spots, but every effort will be made. 

Please select all leagues that you are interested in, as well as pertinent information (available dates, request to split with someone specifically, etc).

Notes about splitting and teams of 5 players:

• Splits arrangements (nights played and fee splitting) will be decided by teams directly.
